Waiver and Liability Release Form

Acknowledgement of Risks

I understand that participation in a candle-making party involves inherent risks including, but not limited to: exposure to hot wax, fragrance oils, melting containers, glass vessels, and various tools; potential spills, slips, burns; and possible allergic or skin reactions to materials used. I acknowledge these risks.

Assumption of Risks and Voluntary Participation

I voluntarily choose to participate in this candle-making event. I understand the nature of the activities and freely assume all risks, both known and unknown, arising from my participation.

Release and Waiver of Liability

To the fullest extent permitted by law, I hereby release, waive, and discharge the Host, venue owner, instructors, affiliates, licensees, assigns, helpers, and their respective agents from any and all claims, demands, actions, or causes of action for injury, loss, damage, or death arising out of or related to my participation in this event.

Safety Agreement

I agree to follow all safety instructions provided by the host or instructors, use all provided protective equipment (gloves, aprons, safety glasses, etc.), and conduct myself responsibly around hot wax, and all materials and tools.

Photo/Media Consent

I understand that there multiple opportunities for photos or videos to be taken during this event, by multiple people, including the Host and candle company, that may be used for promotional or social media purposes.
